CASE REPORT:

Escalating Grandiose Delusions in a 79-Year-Old Male with Presumptive Sporadic Creutzfeldt-Jakob Disease (CJD)


 

Volume 69, Issue 4: “Divine Delusions & Differential Diagnosis”

April 15, 2026  


Presenting Complaint:


Patient recently uploaded an AI-generated image to his personal social media platform depicting himself in flowing white-and-red robes, hands emitting divine light, healing a sick individual in a manner indistinguishable from classical Renaissance paintings of Jesus Christ performing miracles.  When confronted with widespread backlash (including from his own religious base calling it “blasphemy”), the patient deleted the post and offered the following explanation during an impromptu press interaction:


“I thought it was me as a doctor… It had to do with the Red Cross as a Red Cross worker there, which we support… I make people better. I make people a lot better.”



JFIMS Clinical Analysis:  This represents classic progression of prion-related frontal and temporal lobe involvement. 

Stage 1 (Subtle): Occasional word salad, memory lapses (2024-early 2025). 

Stage 2 (Mid): Paranoia, conspiracy fixation, mild grandiosity. 

Stage 3 (Current – Advanced): Full messianic delusion with anosognosia. Patient can no longer distinguish between:

a) Being depicted as the literal Son of God performing miracles, and

b) “Just a humble Red Cross doctor guy, bro, trust me.”

 

The patient’s inability to perceive the obvious Christ-like imagery (glowing hands, robes, halo-like lighting, healing posture) while sincerely believing it was “medical” is pathognomonic for severe cortical prion deposition affecting insight and reality testing.  Differential Diagnosis (updated):  Sporadic CJD — Most likely. Rapid onset, myoclonic potential (still under observation), psychiatric features exploding. 

Vascular dementia with grandiose features — Less likely, no clear stepwise decline reported. 

Just “owning the libs” — Ruled out. Even loyal supporters called this one unhinged. 

Performance art — Patient appeared genuinely confused when corrected.

 

JFIMS Prognosis: Poor. Expect continued escalation — possible future posts of patient as Buddha, Muhammad, or the Moses performing neurosurgery. Caregivers should monitor for further inappropriate religious self-insertion. 

Recommendation: Immediate lumbar puncture for 14-3-3 protein, RT-QuIC assay, and full prion panel. MRI for cortical ribboning and basal ganglia hyperintensities.
